  • Need verifiable proof of experience in stripping, waxing, buffing, carpet extraction, and grout work

Employment Verification Authorization

I authorize On Demand Staffing, or any representative thereof, to contact my present and past employer(s) for the purpose of confirming my length of employment, wages and other relevant data. I also authorize On Demand Staffing, or any representative thereof, to conduct investigative background inquiries on me, to include consumer, criminal, driving, and other reports as needed, on a continuing basis as it relates to my employment. By signing and dating below, I verify and attest that I have personally completed this Application for Employment, and that all information provided is truthful, accurate, and complete. Any applicant found to have provided false, inaccurate, or incomplete information in the Application for Employment will be rejected and may not be considered for future opportunities, and if already employed, may be subject to disciplinary action up to and including termination. On Demand Staffing will not consider partial or incomplete applications. An applicant’s failure to fill out any portion of the application will cause the application to be rejected. Completed applications expire after 30 days. Accordingly, if you are not hired within 30 days of submitting a completed application and you wish to be considered for future openings, you must fill out and submit a new application.

Consent to Test For Drug/Alcohol In the Event of Work-Related Injury or Illness and Acknowledgements Regarding Medical Services and Light Duty

I understand that I am required to immediately report any work-related injury that I may incur as a result of my employment through On Demand Staffing, to the branch manager at On Demand Staffing. I understand that if I am injured, I must seek medical treatment at On Demand Staffing’s designated occupational medical provider. I also understand that if I seek treatment anywhere other than these providers, except in cases of dire medical emergency, I will be responsible for payment of services rendered. I authorize any and all providers of medical treatment deemed necessary for my reported occupational injury or illness to release any medical information acquired in the course of my treatment to On Demand Staffing, their claims administrators, or their agents. I understand that, as part of its regular employment policy, On Demand Staffing requires any employee who suffers a work-related injury or illness to be tested for the presence of drugs and/or alcohol. This testing is to be done at the location where initial treatment for the injury/illness is provided, or as soon as possible thereafter, if it is not taken at the time of initial treatment. Testing is to be conducted in accordance with acceptable medical procedures. I understand that if I refuse to submit to testing, it will be considered as refusal to comply with a reasonable request by my employer and will be cause for dismissal, and may affect my worker’s compensation benefits. I further agree to hold harmless On Demand Staffing, its agents, and clients for any and all consequences arising from my testing positive for the use and/or influence of drugs or alcohol at the time of my injury or illness. If I test positive for the use of drugs or alcohol at the time of injury, I understand that I will be suspended until such time as I can provide a negative drug test, and demonstrate completion of a certified abuse program. If I am offered ‘light duty’ (i.e., work within my prescribed physical limitations), I understand that I must report on time for my light duty assignment. Failure to report at the proper time and/or place of my light duty assignment, or, failure to perform my duties in a diligent manner will be grounds for my immediate dismissal.

Policy Regarding Dispute Resolution

I agree that any disputes arising out of my employment, including any claims of discrimination, harassment or wrongful termination, that I believe I have against On Demand Staffing and all other employment related issues (excluding only those claims arising under the National Labor Relations Act or otherwise within the jurisdiction of the National Labor Relations Board) will be resolved by arbitration as my sole remedy. The American Arbitration Association under its Employment Arbitration Rules shall conduct the arbitration and the decision of the arbitrator shall be final and binding.

Release of Claims Against On Demand Staffing Customers

I understand that On Demand Staffing (“the Company”) provides temporary workers for its customers to work at the customers’ designated job site. In accepting any work assignment, I acknowledge that I am a temporary worker of On Demand Staffing and not an employee of the Company’s customer. If I am ever injured in the course of any work assignment for On Demand Staffing, I agree that I will look only to the Company’s Worker’s Compensation coverage and not the Company’s customer for any recovery. For myself, and on the behalf of my heirs, executor, personal representatives and assigns, I waive, release, and forever discharge any claim that I may now have or that may later accrue against any customer of the Company which directly or indirectly arises out of any injuries which may occur while I am on work assignment for the Company. In signing this Release, I understand that I am not waiving or releasing any claims that I may have against the Worker’s Compensation coverage provided by On Demand Staffing.

Policy Regarding Dispatch Procedure

I understand that I am not required to work any particular day and whether I report in to the On Demand Staffing dispatch hall is always my choice. Whenever I wish to register my availability to work, I will visit the dispatch hall and sign in. I know that On Demand Staffing is not required to find work for me and is not required to contact me in any way in order to make work available to me. If I do not report to the dispatch hall and sign in, On Demand Staffing may assume that I am not available for work on that day. I understand that On Demand Staffing follows a “best match for dispatch” policy and not a “first in-first out” policy. I understand that after receiving a job assignment, I am free on my own time to leave the dispatch hall and do as I wish until the job assignment starts. I understand the importance of never being late for a job assignment. I agree to notify the dispatch Manager of any physical limitation, prescribed medication, previous injury, or pre-existing condition which may impair or limit my ability to perform an offered job assignment. If I have a Repeat Ticket (defined as a request to return to the same job at a later date), I know that I am required to report my availability to On Demand Staffing in the manner indicated by the dispatcher at least one (1) hour before the scheduled start time and that if I do not, then On Demand Staffing may assume that I am not available to return to work.

Daily Employment At-Will

I understand and acknowledge that my employment with On Demand Staffing is on an “AT WILL” day-to-day basis. That is, at the end of the workday, I will be deemed to have quit until I report to the dispatch hall at a later date, receive a new work assignment, and actually commence work on a new assignment. I acknowledge that I am not employed by On Demand while traveling to and from an assignment. I understand that I am employed by On Demand Staffing only during the time period in which I am actually at an assignment and working. Failure to request a new assignment may affect eligibility for unemployment compensation. I understand that merely registering my availability to work does not constitute employment, and I am not re-employed until I actually commence work at a new work assignment. Regardless of my employment status, I understand that I will not be entitled to receive any fringe benefits of any type from On Demand Staffing, including such things as pension plan and vacation. I understand the significance of my exclusion from these programs and irrevocably agree to my exclusion.
